Overview

Practices and promotes the agency’s mission statement and promotes the agency’s guiding principles in every interaction.  Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization, and overall well-being) and to ensure that a high quality of life is maintained.

Responsibilities

Primary duties and responsibilities include but not limited to:

 

  1. Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization and overall well being) and to ensure that a high quality of life is maintained.
  2. Schedules, coordinates, and attends medical appointments and ensures follow through upon return.
  3. Responsible for communicating medical information to the Interdisciplinary team and family (at the Nurse’s discretion).
  4. Assists people with medication administration according to the AMAP policy and procedures.
  5. Completes weekly medication inventory/counts and orders medication accordingly.
  6. Ensures the organization and cleanliness of medication room.
  7. Reviewing Medical books and MARs with the Nurse.
  8. Communicating daily with Nursing and Management about the medical needs of the residence.
  9. Provides transportation for people for medical appointments, shopping, recreational outings and any other transportation needs.
  10. Follows all treatment plans and utilizes all corresponding adaptive and safety equipment needed for each person to promote independence and ensure their health, safety and wellbeing.
  11. Implements and documents all treatment plans and other agency related materials (i.e., residential habilitation goals, medical case notes, communication logs, etc.) to assist people in achieving independence in their selected valued outcomes and to ensure their health, safety and well-being.
  12. Adheres to overtime policy and procedures to ensure that quality support is provided to people each day.
  13. Serves as member of the inter-disciplinary team by attending and participating in all meetings to discuss peoples’ progress and goal achievements and to offer and gather information and ideas.
  14. Ensures a safe and clean home by providing support to people to assist them with the completion of household chores and by participating in said household chores as needed.
  15. Mentors new Direct Support Staff by demonstrating supportive routines and by reviewing training manuals to ensure new employees are sufficiently groomed to complete responsibilities.
  16. Performs all other job-related duties as requested by supervisor.

This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check

 

Qualifications

 

  • High school diploma or equivalent required.
  • Qualified NYS drivers' license.
  • Ability to communicate and understand written and oral directives/information.
