Overview

Practices and promotes the agency’s mission statement and promotes the agency’s guiding principles in every interaction.  Provide ongoing support to individuals in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization, and overall well being) and to ensure that a high quality of life is maintained.

Responsibilities

Primary duties and responsbilities include but not limited to: 

 

  • Follows all treatment plans and provides ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores and socialization) and to ensure that a high quality of health, safety and wellbeing is maintained.
  • Implements and/or documents all agency related materials (i.e., updating sleep charts, reviewing communication logs, training, fire evacuation and Emergency Preparedness plans, schedules, duty checklists) to assist people in achieving independence and to ensure their health, safety and well-being.
  • Ensures a safe and clean home by providing people support, to assist them with the completion of household chores and by participating in said household chores including but not limited to sanitizing  surfaces and completing environmental checks as indicated 
  • Maintains presence at designated post, ready to intervene/assist and monitor throughout the shift.
  • Reports and documents any events/activities that may hinder the health, safety and well-being of persons supported.
  • Reports to the Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect. Must follow all agency incident policy and procedure.
  • Supports people in meal preparation by teaching concepts such as diet adherence, portion control and kitchen safety
  • Successfully completes and maintains AMAP certification and assists people supported with medication administration according to the AMAP policy and procedures. Assists people with medication administration according to the AMAP policy and procedures.
  • Follows toileting schedule as written and documents any soiling accidents that may occur.
  • Adheres to Federal and New York State regulations, agency policy and procedures, the employee handbook and agency and program training requirements
  • Provides transportation/accompanies people for medical appointments, shopping, recreational outings (AM hours) and any other transportation needs, emergency or otherwise, during the overnight hours.
  • Performs all other job related duties as requested by a supervisor. 

 This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check

Qualifications

  •  High school diploma or equivalent required.
  • Qualified NYS drivers' license.
  • Ability to communicate and understand written and oral directives/information. 

Physical Aspects may include but are not limited to lifting and transferring of people  (children and/or young adults)
