Overview

Practices and promotes the agency’s mission statement and promotes the agency’s guiding principles in every interaction.  Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization, and overall well being) and to ensure that a high quality of life is maintained.

Responsibilities

  1. Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization and overall well being) and to ensure that a high quality of life is maintained.

 

       2.Ensures a safe and clean home by providing support to people to assist them with the completion of household chores and by participating in said household chores as needed.

 

        3.Follows all treatment plans and utilizes all corresponding adaptive and safety equipment needed for each person to promote independence and ensure their health, safety and       wellbeing.

 

  1. Supports people in preparing meals by teaching concepts such as diet adherence, portion control and cooking skills.

 

  1. Assists people with medication administration according to the AMAP policy and procedures.

 

  1. Works with residential nurses to ensure accurate and current medical documentation for people supported if maintained.

 

      7.Implements and documents all treatment plans and other agency related materials (i.e., residential habilitation goals, medical case notes, communication logs, etc.) to assist people in achieving independence in their selected valued outcomes and to ensure their health, safety and well-being.

 

      8. Adheres to overtime policy and procedures to ensure that quality support is provided to people each day.

 

     9.Serves as member of the inter-disciplinary team by attending and participating in all meetings to discuss people’s progress and goal achievements and to offer and gather information and ideas.

 

     10. Coordinates medical appointments and provides transportation to people for medical appointments, shopping, recreational outings, and any other transportation needs.

 

     11. Mentors new Direct Support Staff by demonstrating supportive routines and by reviewing training manuals to ensure new employees are sufficiently groomed to complete responsibilities.

 

      12. Communicates with parents about people supported medical appointments and concerns.

 

      13. Performs all other job-related duties as requested by supervisor.

 

This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check

 

 

Qualifications

High school diploma or equivalent preferred.

Valid and qualified NYS drivers' license required. And maintained throughout employment.

Ability to communicate and understand written and oral directives/information. 

AMAP Training is required and must be completed within six (6) months of hire.

First Aid/CPR/SCIP-R Training is preferred.

Minimum one (1) year experience working with intellectual and other development disabilities preferred.
